Almost everything sold for dark spots works on the surface of your skin. Almost nothing about a dark spot happens there.
That is the whole problem, and once you can see it, the pattern people describe to me makes complete sense. Six weeks of a brightening routine. Real improvement. Then it stops, and no amount of continuing moves it further.
Nothing failed. You cleared what had already been delivered, and the factory kept shipping.
Let me show you where the factory is.
Where pigment is actually made
Melanin is produced by cells called melanocytes. They do not sit on the surface. They sit at the very bottom of your epidermis, at the junction where it meets the dermis, several layers below anything you apply.
When one is triggered, an enzyme called tyrosinase starts a chain of reactions that converts an amino acid into melanin. Tyrosinase is the rate-limiting step, which is a useful thing to know, because it means the whole process has a bottleneck worth targeting.
The finished pigment gets packaged into small parcels called melanosomes, and those parcels are handed off to the surrounding keratinocytes. Those keratinocytes then carry the pigment upward as they mature and migrate toward the surface, over roughly a month.
So the mark on your cheek is pigment that was manufactured weeks ago, at the bottom, and has been travelling up ever since.
Treating the top of it is treating the delivery, not the order.
Why routines plateau
This is why the six week pattern is so consistent.
Exfoliation, whether that is an acid or a physical polish, removes pigmented cells that have already arrived at the surface. That works. It is why you see improvement quickly.
But it does nothing to the melanocyte. If the trigger is still present, production continues, and a fresh batch is already partway up the escalator when you start.
Clear the surface faster than you slow production, and you reach a floor. That floor is the plateau.
Which means the answer is a second lever entirely, rather than a stronger exfoliant.
The part that is genuinely counterintuitive
Melanocytes respond to injury.
Inflammation is itself a trigger for pigment production. That is the entire mechanism behind post-inflammatory hyperpigmentation, the mark left behind after a spot, a scratch, a wax, a burn.
So consider what happens when someone attacks a dark spot with a high-percentage acid, a harsh scrub, or an aggressive course of something strong.
They clear some surface pigment. They also inflame the skin. The inflammation signals the melanocytes underneath to produce more.
People do this for months and cannot understand why the spot is winning. The treatment is feeding the thing it is meant to remove.
Which is why every approach to pigment that actually works is a gentle one.
If you have melanin-rich skin, this matters more
Not slightly more. Considerably more.
Deeper skin tones have more melanin and melanocytes that respond more readily. That is a genuine advantage in most respects, and a real vulnerability in this one, because the same responsiveness means inflammation converts into visible pigment faster and more stubbornly.
Most brightening advice on the internet is written without this in mind. Strong acids, high percentages, aggressive resurfacing. That advice is riskier on melanin-rich skin than on pale skin, and it is dispensed identically to both.

Not all dark spots are the same thing
Worth separating, because they behave differently.
Sun spots. Accumulated UV exposure, usually on the face, chest and hands. The most responsive of the three, and the most preventable.
Post-inflammatory marks. Left behind by a breakout or an injury. These fade on their own with time, and the main job is not making them worse in the meantime.
Melasma. Larger, more diffuse, often symmetrical across the cheeks or forehead. It is the most stubborn, it recurs, and it is driven from the inside. Anyone promising to clear melasma quickly is not being straight with you.
On melasma, and where I would actually send you
I want to be clear about the limits of what I do.
Melasma is driven by hormones and metabolic signalling. The trigger is not sitting on the surface, so the surface is not where it gets resolved. A good topical routine supports it, protects it, and keeps you from making it worse, and that is real. It is also not the whole answer, and I am not going to sell you a serum as though it were.
A lifelong friend of mine has built her entire practice around this, and she got there the hard way.
Ariana Juarez spent over a decade chasing topicals, lasers and surface-level fixes for her own melasma. None of it held. What finally moved it was looking inward, at hormones and metabolic function, through functional testing. She now does that work for other people at Alura Wellness, and her tagline is the whole thesis: melasma is metabolic.

The three levers
You need all three at once. Any one on its own is the plateau.
One: take away the signal. UV is the primary instruction telling melanocytes to produce, which makes pigment the one concern where I am strict about sun, and I am not strict about it generally. While you are actively working on a spot, daylight is working against you, and you can spend months treating and re-triggering inside the same twenty-four hours.
So for the season you are working on this: shade, a hat, and proper cover when you are out in it for a stretch. A mineral filter on the days that is not practical. My wider view on daily filter use is in its own piece, and pigment is the clearest exception in it.
Antioxidants sit alongside that, because UV that does get through generates free radicals that also drive pigment production. Two: slow production. This is the lever people skip, and it is the one that breaks the plateau. One note on hydroquinone
You will find it recommended constantly online, so it is worth knowing where it stands. In the United States it is no longer available over the counter. It was removed from the OTC market in 2020 and is prescription-only now.
It works. It also carries a rebound risk, meaning pigment can return more strongly when it is stopped, and it is not something to buy from an unregulated source because a forum recommended it.
If your pigmentation is severe, a dermatologist is the right call, and I will tell you that rather than sell you something.
How long this actually takes
Longer than congestion, and congestion takes two months.
You are waiting for pigment already in transit to finish its journey to the surface and shed, which is weeks on its own, and then for reduced production to show up in the cells forming now.
Twelve weeks before you judge anything. Six months for real change on established spots.
Photograph it. Same spot, same light, same time of day, once a month. Pigment fades so gradually that you will swear nothing is happening while looking directly at improvement. The photographs are the only honest referee, and they are free.
The short version
Pigment is made at the bottom and delivered to the top. Treating the top clears the delivery and leaves the order standing.
Take away the signal. Slow the production. Clear the surface gently, because inflammation is itself an order for more pigment.
Do all three, take photographs, and give it a season.
